- Inscribed within a tabella ansata (w: 0.53 x h: 0.48. top left-hand corner; w: 0.57 x h: 0.48, top right-hand corner, w: 0.88 x h: 0.22, bottom left-hand corner; w: 0.22 x h: 0.16, no edges surviving).
- Letters:
- Lapidary capitals: l. 1, 0.115; l. 2, 0.075; l.3. 0.12; last line 0.085.

- A.D. 153 (titulature)
- Findspot:
- Lepcis Magna: The three first were found in the Forum Vetus, in the Temple of Liber Pater; the findspot of the fourth is not recorded.
